In hospital routine, the person who notices the deviation may not be the same person who follows the next intervention. An observation saved in a conversation, paper or separate file leaves gaps for whoever takes the shift. IQC needs to allow consultation of the result, investigation and decision in the context of the equipment and period.

When recording an incident, describe the evidence observed, the hypotheses considered and the status of the investigation. Differentiate proposed action, executed action and effectiveness verification. The history should allow the next professional to understand what has already been done before repeating interventions or interpreting new controls.
Separate operational priority from technical decision
Release pressure does not replace control acceptance criteria. Define who can investigate, review and escalate a nonconformity. In the demo, simulate a broken rule with control data and confirm how the team finds subsequent records. The system organizes information; the technical decision remains with those responsible.
Evaluate continuity with the contract
Discuss access availability, operation in case of connection failures, support and contingency procedures compatible with service times. Don't assume 24-hour service just because the system is web-based. Record the agreed scope in the contract and organize an internal procedure for routine continuity.
Include the environment in the investigation
Storage and temperature conditions may be relevant to the investigation, depending on the material and process.
